多把吻合器辅助超低位直肠癌手术的临床疗效观察及对患者术后肠道功能及安全性影响

　　[摘要]目的 研究在超低位直腸癌手术过程中应用多把吻合器的临床疗效及其对患者术后肠道功能安全性的影响。方法 选取2010年6月～2015年6月在我院进行治疗的超低位直肠癌患者32例，采用随机数字表法将其分为对照组、研究组，每组16例。研究组中11例患者使用二吻合法进行手术治疗，5例患者使用三吻合法进行手术治疗；对照组使用传统的Miles手术（即经腹会阴联合直肠癌根治术）进行手术治疗。结果 研究组患者的胃肠功能恢复时间和手术时间长于对照组，差异有统计学意义（P<0.05）；两组患者患者术后3 d、1个月和3个月后的每天大便次数比较，差异无统计学意义（P>0.05）；研究组术后生存时间为（10.0±3.2）个月，对照组为（11.0±2.9）个月，两组比较，差异无统计学意义（P>0.05）。结论 在对超低位直肠癌患者进行手术的过程中采用多把吻合器辅助手术的方法，能够完整切除直肠系膜，具有较好的临床疗效，能够充分保障超低位直肠癌患者的保肛成功率及安全性，另外能够尽可能地减少术后患者切缘阳性的现象，较之传统的手术方法能够更好的保证患者术后的生活质量。

　　Clinical observation on the effect of multi - stapling technique on postoperative intestinal function and safety of patients with ultra-low rectal cancer

　　[Abstract]Objective To study the clinical efficacy of multi-stapling technique in the operation of ultra-low rectal cancer and its effect on the safety of postoperative intestinal function.Methods From June 2010 to June 2015，32 patients with ultra-low rectal cancer were selected and randomly divided into control group and study group， 16 cases in each group.In the study group，11 patients were treated by two anastomosis and 5 patients by three anastomosis.The control group was treated with conventional miles operation （i.e，transabdominal and perineal resection of rectal cancer）.Results The recovery time and operation time of gastrointestinal function in the study group were longer than those in the control group （P<0.05）.There was no significant difference between the two groups （P>0.05） in the number of daily defecation after 3 d，1 month and 3 months.The survival time was （10.0±3.2） months of the study group and （11.0±2.9） months in the control group，there was no significant difference the two groups（P>0.05）.Conclusion In the course of operation for patients with ultra-low rectal cancer，application of multi-anastomat as an assistance during the operation can completely removemesorectum in a great clinical effect by fully ensuing the success rate and safety of anal preservation.Inaddition，itreduces the positive phenomenon of incisal edge in these postoperative patients，and better guarantees the quality of life in these patients after surgery compared with the traditional operation method.
