Epworth嗜睡量表联合家庭便携式睡眠监测对成人阻塞性睡眠呼吸暂停综合征的诊断价值

【摘要】 目的 探討Epworth嗜睡量表(ESS)联合家庭便携式睡眠监测(PM)对成人阻塞性睡眠呼吸暂停综合征(OSAS)的诊断价值。方法 124例疑诊为OSAS患者, 均接受ESS和PM以预测OSAS高风险, 并进行整夜多导睡眠监测(PSG), 根据PSG检测结果将患者分为单纯鼾症组(58例)和OSAS组(66例), 分别比较两组以及不同程度OSAS患者的ESS评分、体质量指数(BMI)、呼吸紊乱指数(RDI)、呼吸暂停低通气指数(AHI), 分析ESS评分、RDI与AHI的相关性。结果 单纯鼾症组患者ESS评分、AHI以及RDI水平明显低于OSAS组, 差异具有统计学意义(t=3.903、14.783、13.789, P<0.05)。两组患者的BMI水平比较差异无统计学意义(t=0.733, P>0.05)。66例OSAS患者根据严重程度分为轻度32例、中度19例、重度15例。不同程度OSAS患者的ESS评分、AHI及RDI水平比较, 差异具有统计学意义(P<0.05)。不同程度OSAS患者的BMI水平比较差异无统计学意义(P>0.05)。经相关性分析显示, ESS评分、RDI水平与AHI均呈正相关(r=0.465、0.674, P<0.05)。结论 Epworth嗜睡量表联合家庭便携式睡眠监测在成人阻塞性睡眠呼吸暂停综合征中具有较高的诊断价值, 简明易用, 同时也可作为评估病情严重程度的重要指标, 值得在临床推广应用。

【Abstract】 Objective To discuss the diagnostic value of Epworth somnolence scale (ESS) combined with family portable monitoring (PM) for adult obstructive sleep apnea syndrome (OSAS). Methods A total of 124 suspected OSAS patients underwent ESS and PM to predict high risk of OSAS, and they all received all night polysomnography (PSG). According to the results of PSG, patients were divided into simple snoring group
(58 cases) and OSAS group (66 cases). The ESS scores, body mass index (BMI), respiratory disturbance index (RDI) and apnea hypopnea index (AHI) were compared between the two groups and OSAS patients in different degrees. The correlation between ESS score, RDI and AHI was analyzed. Results Simple snoring group had obviously lower ESS score, AHI and RDI level than OSAS group, and the difference was statistically significant (t=3.903, 14.783, 13.789, P<0.05). Both groups had no statistically significant difference in BMI level (t=0.733, P>0.05). 66 OSAS patients were divided by severity into 32 light cases, 19 moderate cases and 15 severe cases. There was statistically significant difference in ESS score, AHI and RDI level in OSAS patients in different degrees (P<0.05). There was no statistically significant difference in BMI level in patients in different degrees (P>0.05). According to correlation analysis, ESS score and the level of RDI were positively correlated with AHI (r=0.465, 0.674, P<0.05). Conclusion Combination of Epworth somnolence scale and family portable monitoring shows high diagnostic value in adult obstructive sleep apnea syndrome. It is simple and easy to use, and it can also be used as an important index to assess the severity of illness. It is worthy of clinical promotion and application.
