社区中医护理对老年类风湿关节炎患者生活质量的影响

　　[摘要]目的 探讨社区中医护理对老年类风湿关节炎患者生活质量的影响。方法 收集2014年1月～2016年1月我社区老年风湿关节炎患者92例临床资料，依据患者的意愿分为对照组和实验组，每组46例，对照组患者采用临床常规方法治疗和护理，实验组患者在对照组患者治疗和护理的基础上实施社区中医护理，用SF-36生存质量量表对患者的生存质量进行评估并比较两组患者的康复效果。结果 社区中医护理后实验组的生存质量评分高于对照组，差异有统计学意义（t=8.694，P=0.000）；实验组总有效率97.40%，显著优于对照组的77.10%，差异有统计学意义（P<0.01）。结论 开展社区中医护理对老年人类风湿关节炎生存质量有着积极的作用，值得临床上推广应用。

　　Influence of community traditional Chinese medicine nursing on quality of life in elderly patients with rheumatoid arthritis

　　[Abstract]Objective To explore the influence of community traditional Chinese medicine （TCM） nursing on the quality of life （QOL） in elderly patients with rheumatoid arthritis.Methods The clinical data of 92 senile patients with rheumatoid arthritis in our community from January 2014 to January 2016 were collected.According to patient′s will，they were evenly divided into the experimental group and the control group，with 46 cases in each group，in the control group，the clinical conventional routine method and nursing was used，in the experimental group，community TCM nursing was applied on the basis of the control group.The patient′s QOL was assessed by SF-36 scale，and the rehabilitation effect was compared between the two groups.Results The quality of life score of the experimental group was higher than that of the control group， the difference was statistically significant （t=8.694，P=0.000）.The total effectiveness rate in the experimental group was 97.40%，which was significantly better than that of the control group accounting for 77.10% with a statistical difference （P<0.01）.Conclusion Implementation of the community TCM nursing has a positive effect on QOL in the elderly patients with rheumatoid arthritis，and it is worthy of clinical application.

　　類风湿关节炎属于一种以侵蚀性、对称性为基本特征表现的关节炎症反应类疾病，同属也是慢性、全身性自身免疫性疾病的一种。本病呈全球性分布，是目前临床乃至全社会公认的一种导致人类劳动力丧失和致残的一个非常主要的原因。根据相关领域所进行的研究资料显示[1]，该病的发病率为0.32%～0.36%，由于类风湿关节炎需终身治疗，治疗时间长，预后效果往往不佳[2-3]。该疾病所具有的高致残性，可以使广大患者在患病时候的短时间内迅速丧失劳动能力，生活不能自理，使情绪出现异常，对生活质量造成严重影响[4]。故而在进行治疗的过程后，需给予适当的社区护理，以改善患者的情绪、生活质量。有研究显示，社区中医护理应用于类风湿关节炎中，能够显著改善患者的情绪、康复效果及生活质量[5]。现笔者为探讨社区中医护理对类风湿关节炎患者预后及生活质量的影响，将类风湿性关节炎患者分为两组，分别应用常规护理、社区中医护理，比较其康复效果及生活质量，现报道如下。
